Job summary

Fuze HR Solutions is seeking a Registered Practical Nurse (RPN) for a permanent, full-time position in Kingston, Ontario. The role supports a community-based primary care team with hands-on nursing care, health education, client support, and clinical coordination within an interdisciplinary setting.

The successful candidate will have 3+ years of nursing experience (community or primary care preferred), current CNO registration, and experience with electronic health records.

Qualifications

  • Must be registered with the College of Nurses of Ontario (CNO).
  • Minimum 3 years of nursing experience, preferably in community/primary care.
  • Experience with electronic health records and team-based care.

Responsibilities

  • Provide nursing care and support to clients in a primary care setting.
  • Conduct client assessments, triage, follow-up, and care planning.
  • Support physicians and other providers during procedures.
  • Perform basic lab and nursing tasks (phlebotomy, urine, glucometer, injections).
  • Deliver health education, screening, referrals, and follow-up.
  • Maintain accurate documentation in electronic health records.
  • Participate in team meetings and quality improvement initiatives.
  • Assist with infection control, privacy, and safety standards.

Registered Practical Nurse - Primary Care

Location: Kingston, ON | Position Type: Permanent, Full-Time | 35 hours/week | Compensation: $27–$31/hour

About the Opportunity

Our client is a growing community-based healthcare organization providing accessible, client-focused primary care services. They are seeking a Registered Practical Nurse (RPN) to join an interdisciplinary team and provide hands-on nursing care, health education, client support, and clinical coordination.

This role is well suited to an RPN with community nursing experience who is comfortable working with diverse and underserved populations and contributing to a collaborative primary care environment.

Key Responsibilities
  • Provide nursing care and support to clients within a primary care setting and in accordance with professional standards.
  • Conduct client assessments, telephone triage, follow-up, and ongoing care.
  • Support physicians and other healthcare providers during clinical procedures.
  • Perform basic laboratory and nursing procedures, including phlebotomy, urine testing, glucometer testing, wound care, injections, immunizations, and other treatments within scope.
  • Provide health education, screening, referrals, and client follow-up.
  • Work under medical directives as required.
  • Maintain accurate and timely documentation within electronic health records.
  • Participate in case conferences, chart reviews, team meetings, and quality improvement initiatives.
  • Maintain clinical, laboratory, and program supplies.
  • Follow infection prevention, occupational health and safety, privacy, and other applicable standards.
  • Support harm reduction initiatives, including distributing supplies and providing education.
  • Collaborate with an interdisciplinary team to provide a positive and seamless client experience.
  • Participate in occasional extended hours or other duties as required.
Qualifications
  • Registered Practical Nurse in good standing with the College of Nurses of Ontario (CNO).
  • 3+ years of nursing experience, ideally within a community, primary care, public health, or hospital environment.
  • Experience working under medical directives.
  • Experience supporting diverse, underserved, or equity-seeking populations.
  • Strong clinical assessment, documentation, communication, and organizational skills.
  • Proficiency with electronic health records and standard computer applications.
  • Ability to work effectively within an interdisciplinary team.
  • Current CPR and First Aid certification.
  • Satisfactory criminal record and vulnerable sector screening.
Assets
  • Primary care or community health experience.
  • Experience with harm reduction programs.
  • Trauma-informed care experience.
  • Smoking cessation training.
  • Experience supporting program development or quality improvement initiatives.
  • Experience facilitating client education or group sessions.
