Job summary

Cogir Senior Living is seeking a Registered Practical Nurse to provide patient-centered nursing care for residents in our Canadian senior living communities in Halton Region. You will coordinate with a multidisciplinary team to deliver personalized services and uphold high standards of care.

The role requires current RPN registration, strong clinical knowledge, and experience with geriatric care. Competitive benefits, ongoing training, and growth opportunities are offered in a supportive team

Qualifications

• Current registration as a Registered Practical Nurse in the applicable province.
• Demonstrated sound clinical knowledge.
• Experience with seniors and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/long term care an asset.
• Strong drive, respect and understanding of the demands of the role.
• Teamwork and customer service with ability to build trusted relationships.
• Strong written and oral communication and critical thinking skills.
• Organizational and time management skills.
• Computer proficiency and experience with Microsoft Office applications.
• Current Basic Cardiac Life Support and First Aid certification required.
• Current (within six months) Vulnerable Sector Check or willingness to obtain one.

Responsibilities

• Provide planned, patient-oriented, goal-directed nursing care in accordance with provincial standards.
• Be an integral part of a multi-disciplinary team with ongoing communication.
• Implement and evaluate the care plans and communicate changes as required.
• Complete and ensure resident assessments, care plans and documentation are accurate and up to date.
• Teach, delegate and direct unregulated care providers with coaching and mentorship.

How YOU make a difference

RPNs are responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with the provincial standards of care.

RPNs are an integral part of our multi-disciplinary team who build trusted relationships with our residents, families and teams with continuous ongoing and effective communication.

RPNs implement and evaluate the effectiveness of the care plans and make changes as required.

RPNs complete and ensure resident’s clinical care assessments, care plans and documentation are accurate, up to date and communicate changes as required.

• Strong written and oral communication skills and critical thinking skills
• Demonstrated organizational and time management skills
• Computer proficiency and experience with Microsoft office applications
• Current Basic Cardiac Life Support and First Aid certification required
• Current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
