Job summary

Connective Support Society in Canada seeks outreach staff to assist clients referred by Community Living BC, helping them adjust to and succeed in community life.

Role involves working with clients, families, and professionals to plan interventions, coordinate housing and daily living supports, and use crisis response skills when needed. Flexible hours include evenings and weekends, with a focus on client needs.

Qualifications

• Diploma in Social Services and 1 year of relevant experience preferred.
• Experience working with vulnerable populations.
• Ability to respond effectively in crisis situations.

Responsibilities

• Works with the client, researches background material, and seeks input from other professionals and family members to assess needs and intervention possibilities.
• Works with the client to consider issues and agree on an intervention and support plan with measurable short and long-term goals.
• Ensures the client’s intervention and support plan addresses daily living issues (housing, nutrition, clothing, hygiene, leisure, access, employment, education).
• Supports clients in various environments, including exposure to bedbugs, animals, needles, blood, lice, human waste, hoarding, etc.
• Interacts with professionals, family, and organizations to ensure services and treatments support long-term goals.
