Qualifikationen

• Diploma in a related human/social service field.
• 1 year of related experience preferred.
• Experience with harm reduction approaches is desirable.

Aufgaben

• Participates in assessment, goal setting, and program planning for residents.
• Evaluates client needs and develops short-term plans with resident participation.
• Works with staff and community partners to inform individual support plans.
• Assists residents to function more independently in daily life and community activities.
• Administers medication according to policy and maintains health and safety standards.
• Completes regular walk-throughs, documents progress, and reports to families as needed.
• Organizes meals and participates in meal planning with residents.
