Job summary

Connective in Coquitlam is seeking a dedicated outreach support worker to assist clients referred by funders, helping them adjust to life in the community, access services, and navigate housing, nutrition, and health needs.

You will collaborate with the care team, research background material, develop intervention plans with measurable goals, maintain thorough case notes, and report progress while respecting policies and program standards.

Qualifications

Diploma or Certificate in relevant field (one year related experience).
Crisis counselling experience.
Experience with clients with history of conflict with the law and psychiatric involvement.
Knowledge of community services in Lower Mainland and Fraser Valley.
Experience in assessment, case/care plan development and implementation.
Ability to deal with potential violence.
Excellent written and verbal communication skills in English.

Responsibilities

Provides assistive outreach support to referred clients and helps with adjustment to community life.
Works with client to create an intervention and support plan with measurable goals.
Ensures daily living needs (housing, nutrition, clothing, hygiene, employment, education) are addressed in the plan.
Addresses ongoing legal, mental, and physical health issues within the plan.
Delivers a range of interventions to help clients achieve established goals.
Maintains extensive client records and completes monthly progress reports.
