Role and Facility Information

The REACH UPCC focuses on triage and delivery of urgent primary care services to any patient during operating hours. It provides urgent primary care from 8am-10pm Monday to Saturday and 9am-5pm on Sundays, regardless of attachment to another primary care provider.

The program emphasizes urgent primary care and addresses social determinants of health, connecting patients to care within the Primary Care Network surrounding REACH UPCC, functioning as a hub and integrator.

The healthcare team includes physicians, nurse practitioners, advanced practice RNs, social workers, and support staff. Oscar is used as the EMR system.

Compensation

• Type: Clinical Service Contract
• Estimated Remuneration: Mon-Fri before 6pm $170/hr; evenings after 6pm/weekends and holidays $201/hr
