The Canadian Mental Health Association Waterloo Wellington (CMHA WW) is hiring a Support Coordinator (FACT) to join a multidisciplinary team in a regular full-time role based in Kitchener. The position focuses on assessment, short-term monitoring, and coordinating supports for individuals with mental health needs, with an emphasis on recovery-oriented planning and assertive outreach.

Qualifications

• Undergraduate degree/diploma in psychology, social work or a relevant health or social science field
• Minimum of three years’ experience providing community based mental health/addictions services
• Demonstrated assessment and crisis intervention skills
• Strong interpersonal and communication skills (verbal and written)
• Knowledge of Recovery philosophy and community resources
• Valid driver’s license and access to transportation

Responsabilités

• Facilitates person-centered planning to help individuals choose, obtain and maintain life choices
• Identifies and plans for support needs over areas like employment, housing, education and daily living
• Develops and implements service agreements and coordinates supports
• Advocates with and on behalf of people supported and engages family/community when possible
• Coordinates meetings with community partners and participates in committees as needed
