Canadian Mental Health Association Waterloo Wellington is seeking a Service Coordinator (Youth Outreach Worker) for the Wellington-Guelph hub. The role focuses on case management, youth engagement, and collaboration with a multidisciplinary team to support youth aged 12–25, including in-person and virtual services.

The position is full-time, on-site in Palmerston, Ontario, with a 35-hour workweek and some evenings or Saturdays. Competitive benefits and HOOPP pension are offered.

Qualifications

• Undergraduate degree/diploma preferred in psychology, social work or relevant health field.
• 3–5 years of healthcare or social services experience.
• 3–5 years of experience with children, youth, families and caregivers.
• Experience facilitating groups or with diverse participants is an asset.
• Knowledge of youth health, substance use, trauma and recovery.
• Familiarity with MS Office and email; willingness to learn new systems.

Responsabilités

• Promote youth engagement and development in care planning and goal setting.
• Create a safe, welcoming hub atmosphere for youth and families.
• Engage with clients individually and as group facilitator.
• Mentor youth to navigate mental health systems and services.
• Support completion of standardized measures to improve youth outcomes.
• Coordinate care transitions within the Hub Network and connect to community resources.
• Plan and co-facilitate skills activities and drop-in programming.
• Document client interactions and maintain electronic records.
