Job summary

CMHA Waterloo Wellington is hiring a Service Coordinator (Youth Outreach Worker) for the Youth Wellness Hubs Ontario, Wellington-Guelph. This full-time, on-site role is based in Palmerston, Ontario, and runs through November 26, 2027, with a 35-hour workweek (Mon-Fri 10am–6pm) and occasional evenings and Saturdays.

Qualifications

Undergraduate degree or college diploma in a relevant field.
3–5 years of healthcare or social services experience.
Experience with youth, families and caregivers is an asset.
Knowledge of youth mental health, substance use and trauma-informed care.
Ability to document interactions and manage data electronically.

Responsibilities

Provide in-person/virtual case management for youth 12–25.
Navigate community resources and support continuity of care.
Offer one-on-one and group navigation and mentorship to youth.
Engage families and coordinate with a multidisciplinary team.
Document client interactions and complete data entry on schedule.
