We are recruiting for a SUPPORT COORDINATOR (FACT)

Support will be provided on an assertive outreach basis.

Flexible Assertive Community Treatment (FACT) is a recovery-oriented, evidence-based best practice. A FACT team is comprised of amultidisciplinary team of clinicians providing monitoring and treatment to clients with severe and persistent mental illness (SPMI).
