ConnectWell Community Health is seeking a dynamic Nurse Practitioner (0.7 FTE) to join the interdisciplinary team providing primary health care to rostered clients at the ConnectWell Community Health – Lanark site (24.5 hrs/week). Position Summary:
ConnectWell Community Health is a fully accredited, community-governed, not-for-profit organization offering primary health care, health promotion, illness prevention, developmental, and social services. Our salaried interdisciplinary team members work collaboratively to address the health and wellbeing needs of the community, practicing to their fullest scope.
Responsibilities:
- Provide primary health care services in clinic and community settings, emphasizing health promotion, client education, and self-management.
- Adhere to the Standards of Practice established by the College of Nurses of Ontario.
Qualifications:
- RN (EC) license/NP Primary Health Care Certificate.
- Current and unrestricted registration as an RN (EC) with the College of Nurses of Ontario.
- Three years of experience in a nurse practitioner role in primary care or community setting (preferred).
- Effective team collaboration skills.
- Strong clinical skills and community-based health care knowledge, preferably in rural or disadvantaged populations.
- Experience in health education and promotion programs.
- Ability to work evenings and provide on-call telephone services on a rotation basis.
- Valid driver’s license and vehicle.
- Attention to detail, prioritization, and time management skills.
- Excellent communication skills.
Reporting Relationship:
The successful candidate will report to the Primary Care Manager.

Employment Details:
