Job Title: Patient Navigator

Accountable To: Executive Director

Status: Permanent Full Time 1.0 FTE (35 hours/week)

Date: August 2025

The Ottawa Nurse Practitioner-Led Clinic (NPLC) is a not-for-profit, community-governed health care organization dedicated to providing accessible, trauma-informed, and equitable primary care to individuals who face barriers to accessing traditional health services. We serve a diverse population, with a focus on clients impacted by mental health, substance use, poverty, and systemic inequities.

The Patient Navigator plays a key role in helping clients access the care and services they need both within the Ottawa NPLC and across the broader health and social service system. As part of an interprofessional team, the Patient Navigator supports clients in overcoming barriers to care, coordinating referrals, advocating on their behalf, and ensuring that services are person-centered, culturally safe, and trauma-informed.

· Support patients in understanding and accessing primary care, mental health, addictions, housing, and social support services.

· Facilitate referrals to internal providers (e.g., NPs, RNs, social workers) and external partners.

· Follow up with patients to ensure care plans are understood and progressing.

· Assist patients in completing forms and navigating public systems (e.g., OW, ODSP, referrals, housing applications).

· Support patients in obtaining or replacing essential documentation such as health cards (OHIP), government-issued ID, and other required paperwork.

· Advocate for patients within health and social service systems.

· Identify and address barriers to care (e.g., language, transportation, stigma).

· Provide warm hand-offs and accompany patients to appointments when necessary.

· Build and maintain relationships with local service providers and community partners.

· Maintain an up-to-date knowledge base of available community resources.

· Attend community meetings and outreach events on behalf of the clinic as needed.

· Support clients in developing self-advocacy and health literacy skills.

· Promote client empowerment and participation in care planning.

· Provide trauma-informed and culturally safe support in all interactions.

· Maintain accurate and timely documentation in the EMR.

· Participate in team huddles, care conferences, and quality improvement initiatives.

· Contribute to evaluation and data tracking for service effectiveness.

· Diploma or degree in Social Work, Community Services or a related health/human services field.

· Minimum 2 years of experience in case management, care coordination, or patient navigation.

· Demonstrated experience working with marginalized populations, including individuals experiencing homelessness, substance use, or mental illness.

· Strong knowledge of local community resources and health/social services in Ottawa.

· Familiarity with trauma-informed care, harm reduction, and anti-oppressive practices.

· Excellent communication, interpersonal, and organizational skills.

· Ability to work independently and as part of a collaborative, interprofessional team.

· Proficient in using electronic medical records and Microsoft Office applications.

· Bilingualism (English/French) and/or additional languages are considered strong assets.

Annual Salary range- TBD
