Job summary

SaskAbilities seeks a part-time Rehabilitation Worker to coordinate and facilitate life enrichment activities in clients' homes and in the community, enhancing quality of life for individuals living with dementia. You will support health aging at home and work with care partners to plan person-centered care.

Reporting to the Dementia Friendly Coordinator, you will collaborate with team members, manage time effectively, and utilize Microsoft Office tools to document outcomes and observations.

Qualifications

  • Diploma in Rehabilitation or equivalent (2 year).
  • Valid driver’s license and reliable vehicle for work.
  • First Aid/CPR/AED certification or ability to obtain within 3 months.
  • Experience with dementia or disabilities preferred.

Responsibilities

  • Coordinate and facilitate life enrichment activities in homes and in the community.
  • Provide supportive living services to support health aging at home.
  • Build trusting relationships with care partners to support quality of life.
  • Collaborate with team members to meet clients' needs.

Employment Type: Permanent, Part-time .5 FTE position (Working 18.5 hours per week within a varied schedule)

Compensation

Compensation Pay Class 15 (range of $29.01 to $36.05 per hour), plus Extended Health and benefits package

Duties and responsibilities

Duties include:

  • Coordinate and facilitate individual and group life enrichment activities in personal homes and in the community, enhancing the quality of life for individuals living with dementia
  • Coordinate and facilitate supportive living services to clients to support health aging at home
  • To build trusting relationships with care partners to understand the needs of their loved ones and how to best support their quality of life through person-centered planning
  • To work collaboratively with other team members to ensure the needs of the clients and care partners are being met
Qualifications
Education
  • Rehabilitation Worker Diploma or equivalent (2 year diploma), Personal Care Aide
  • Valid Class 5 Driver’s Licence and a reliable vehicle that can be used for work and transporting clients
  • Valid First Aid/CPR/AED certification, or obtain within three months
  • TLR/PART training
Experience
  • Experience working with people living with dementia and/or disabilities
  • Working independently and within a team
  • Problem-solving, being empathetic, patient and resilient
  • Ability to work cooperatively with care partners and referral sources
Skills
  • Excellent organizational and time management skills
  • Strong interpersonal, networking, and relationship-building skills
  • Computer literacy in Microsoft Office, Outlook, and database including file recordings and statistical reporting
