Job summary

Qualifications

Responsibilities

• Collaborate with patients and families to develop and modify care plans.
• Coordinate nursing care, set priorities and delegate tasks to support staff.
• Collaborate with interprofessional team to ensure comprehensive care planning.
• Educate patients and families about illness prevention and health maintenance.
