Job summary

Carefor is Eastern Ontario’s largest home care and community support services not-for-profit, proudly serving Pembroke-Renfrew County. A Personal Support Worker (PSW) provides personal care and homemaking, collaborating with health care teams to support independence and well-being.

Under supervision of the Client Services Manager, the PSW works within policies, maintains safety, and participates in ongoing training.

Qualifications

• High school education preferred.
• Must hold a PSW or Health Care Aide certificate.
• Current Standard First Aid certificate required.
• Current CPR Level C certificate required.
• Experience with elderly or ill individuals in community setting.
• Ability to work in a multidisciplinary team.
• Strong communication and problem-solving skills.

Responsibilities

• Assist or provide care according to the Resident Care Plan.
• Observe resident for changes in behaviour or health status.
• Chart care and report to the care team.
• Evaluate the need for changes in the care plan and make recommendations.
• Contribute to development of the personal support care plan.
• Plan and organize each day’s activities and priorities.
• Complete documentation per Carefor policies.
• Review care notes and communications to stay current.
