Job summary

Carefor Health & Community Services in Eastern Ontario seeks a dedicated Personal Support Worker (PSW) to provide essential in-home care and support to clients. You will assist with daily living activities, promote independence and dignity, and work under the supervision of a registered nurse.

Qualifications

• PSW certificate or equivalent qualification.
• Valid driver’s license and access to a vehicle.
• Clear vulnerable sector police check.
• Experience in home care, retirement homes, or long-term care (asset).
• Ability to work independently and manage multiple tasks in a fast-paced environment (asset).

Responsibilities

• Provide client-centred support and assistance with activities of daily living (ADLs).
• Observe and report changes in client condition to the supervising nurse or care team.
• Assist with medication reminders or administration per care plan.
• Perform light housekeeping, laundry, and meal preparation to maintain a safe environment.
• Engage clients in conversation and activities to reduce isolation and promote well-being.
• Document care provided and communicate with clients, families, and team members.
• Follow PPE protocols and infection control practices.
• Collaborate with nurses, supervisors, and other health care professionals.
• Provide basic ostomy and catheter care; assist with end-of-life and respite care where appropriate.
