Job summary

COGIR Real Estate is seeking a Personal Support Worker to provide compassionate in‑home and on‑site care for residents in Ontario. The role reports to the Health and Wellness Manager and requires adherence to care plans, safety protocols and confidentiality.

You will assist with daily living activities and support residents to maintain independence. The position emphasizes dignity, respect and responsiveness, with LSOTA standards, BCLS, and First Aid certification required.

Qualifications

• Graduate of Personal Support Worker program or provincial equivalent with minimum 600 hours theory & practicum.
• Experience with seniors in community health services.
• Strong written and oral communication skills.
• Demonstrated organizational and time management skills.
• Strong interpersonal and customer focus skills.
• Demonstrated ability to provide personal care duties.
• Current Basic Cardiac Life Support and First Aid certification.
• Ability to perform lifting/transferring tasks.

Responsibilities

• Adhere to the plan of care as directed by the supervisor.
• Facilitate residents' move-in and transfers when applicable.
• Observe and report on care/service using appropriate forms or tools.
• Assist residents with personal care, hygiene and activities of daily living.
• Observe changes in residents' condition and report to supervisor.
• Escalate medical emergencies as directed.
• Plan and prepare nutritious meals as required.
• Assist with ambulation, mobilization, transferring and adaptive aids.
• Provide cognitive, social, emotional and psychological support to residents and families.
• Maintain resident information confidentiality.
