AgeCare seeks a Licensed Practical Nurse (LPN) to join our senior care team on site. You will deliver goal-directed nursing care, conduct assessments, develop care plans, and collaborate with physicians and the care team to ensure residents receive compassionate, high-quality services in a supportive environment.

Reporting to the Director of Care, you will manage medications, educate residents and families, and participate in care conferences.

Qualifications

• Current LPN registration in the applicable province.
• Experience in senior or long-term care preferred.
• CPR and First Aid certification current.
• Proficiency with electronic documentation and Microsoft Office.

Responsibilities

• Collaborate with care team using a person-centered approach.
• Maintain and communicate resident assessments, care plans, and documentation.
• Implement and evaluate care plans; adjust as required.
• Participate in resident care conferences and scheduling.
• Provide direct nursing care and administer medications per policies.
• Coordinate care plans with physicians and team members.
• Delegate tasks to unregulated care providers and supervise as needed.
• Respond to medical emergencies and support infection control initiatives.
