Job summary

AgeCare is seeking a Licensed Practical Nurse (LPN) to join our team and deliver goal-directed, patient-centered nursing care to residents. Reporting to the Director of Care, you will conduct assessments, develop and implement care plans, administer medications, and coordinate with physicians and caregivers to ensure dignity and quality of life.

You bring current LPN registration, experience in senior or long-term care, strong communication, and proficiency with electronic documentation;

Qualifications

• Current LPN registration in the applicable province.
• Experience in senior or long-term care (preferred).
• Strong clinical knowledge and care coordination skills.
• Excellent communication, critical thinking, and time management.
• CPR and First Aid certification.
• Proficiency with electronic documentation and Microsoft Office.
• Completion of a Vulnerable Sector Check (or willingness to obtain).

Responsibilities

• Work collaboratively with the care team using a person-centered care approach.
• Conduct and maintain accurate resident assessments, care plans, and documentation; communicate updates as needed.
• Implement and evaluate care plans, adjusting as required.
• Provide direct nursing care and administer medications and treatments in compliance with policies and regulations.
• Coordinate care plans with physicians, team members, and service providers.
• Respond to significant changes in residents’ conditions and manage medical emergencies.
• Oversee emergency response in the absence of senior leadership.
• Address resident/family concerns and ensure timely follow-up or escalation.
