Job summary

Bria Communities LP in Delta, BC seeks a qualified Licensed Practical Nurse to provide direct nursing care in a long-term care setting. You will administer medications, participate in care planning, and monitor resident health status.

Qualifications

Must be registered in good standing with the BC College of Nurses and Midwives (BCCNM).
Graduation from an accredited Practical Nurse Program.
Minimum of two (2) years nursing experience in long term care is preferred.

Responsibilities

Provide direct nursing care, including administration of medications, medical treatments and documented procedures as per provincial regulations and facility policies.
Evaluate the effectiveness of nursing care plans and treatments.
Assist with admissions, discharges and transfers of residents.
Ensure the appropriate clinical assessment of residents is completed and followed up on through a care plan or planned intervention (e.g., Braden, SLUMS, Morse Fall Risk, vitals, medication reviews, care conferences).
Develop and implement individualized care plans and ensure the plans are implemented.
Liaises with Residents’ families, updating them with changes in health status, medications, or personal care needs.
