AgeCare in British Columbia is seeking a Licensed Practical Nurse (LPN) to join our interdisciplinary team, delivering goal-directed, patient-focused care and maintaining accurate assessments and care plans. Requirements include current LPN registration in the applicable province, CPR/First Aid certification, and proficiency with electronic documentation.

Qualifications

• Current registration as Licensed Practical Nurse (LPN) in the applicable province/state.
• Experience in senior care or long-term care preferred.
• CPR and First Aid certification.
• Proficiency with electronic documentation and Microsoft Office.

Responsibilities

• Collaborate with care team using a person-centered approach and maintain resident assessments and care plans.
• Administer medications and treatments in accordance with policies and regulations.
• Coordinate care plans with physicians and service providers; delegate tasks to unregulated care providers.
• Respond to changes in residents' conditions and assist with emergencies.
• Support infection prevention, safety initiatives, and quality improvement.
