Job summary

Providence Health Care in British Columbia is seeking a Licensed Practical Nurse (LPN) to provide compassionate, patient-centred nursing care to an assigned group of residents, in collaboration with the interdisciplinary team.

Reporting to the Patient Care Manager, the LPN participates in care planning, assessments, medication administration, and supports orientation of staff, with opportunities for professional development and growth.

Qualifications

• Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience
• Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
• Current basic cardiac life support certification

Responsibilities

• Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities
• Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed
• Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team
• Administer medications per protocol, monitor effects, and maintain communication with the team
• Utilize evidence-based resources to stay updated on nursing practices
• Respond to incidents such as codes, injuries, spills, and alarms, following procedures
• Assist with staff orientation, demonstrating procedures and providing support
• Address patient/resident and family learning needs, provide educational materials, and make referrals
• Attend care planning meetings, provide input, and advocate for patients/residents
• Collaborate with the team to develop and evaluate individualized care plans
• Assist with discharge planning based on patient status and external supports
• Operate and clean equipment, report malfunctions, and handle hazardous materials. Perform clerical duties such as answering phones, faxing, and maintaining records
