Qualifications

• ASL proficiency is highly preferred.

Responsibilities

• Support clients to work toward their Individual Support Plan goals and measurable outcomes.
• Perform thorough shift exchange with the previous shift.
• Administer medications using Catalyst system and verify arriving medications.
• Maintain a clean, safe, and comfortable environment for clients.
• Assist guardians and communicate with them as directed by the site supervisor.
• Support clients' care needs (hygiene, dressing, meals, etc.).
• Monitor and support clients throughout the shift.
• Complete and maintain required documentation.
• Plan, prepare and document meals and snacks for clients.
• Perform weekly admin tasks and occupancy reports.
• Carry out daily/weekly household duties to ensure safety and comfort.

Description

Hours of work: Temp. 4 rotational shifts 12p-8p (64 hrs. biweekly) (CSW I- CR21)

What You’ll Do:

• Daily supporting clients to work towards their Individual Support Plan - client goals and identified measurable outcomes.
• Performing a thorough and complete shift exchange with previous shift.
• Using Catalyst system while administering medications to clients on time and checking/receiving medication that arrive while on shift.
• Maintaining a clean, safe, and comfortable environment for clients.
• Making Guardians feel welcome into the client’s residences and communicating/interacting with guardians of clients as directed by the site supervisor.
• Supporting client’s care needs such as: hygiene, dressing, making beds, toileting, etc.
• Assisting and monitoring clients through the entire shift.
• Completing, reading, and maintaining all required documentation.
• Planning for, preparing, and documenting meals and snacks for clients.
• Performing other administrative duties such as weekly confirmations, order money, end of the month client service updates, send in occupancy reports, and any other assigned tasks.
• Performing daily and weekly household duties such as: dusting, washing walls, vacuuming, cleaning, laundry, washing windows, shovelling snow, cleaning bathrooms, washing dishes, etc. ensuring a safe and comfortable environment for clients.
