Responsibilities

  • Support clients daily to work toward Individual Support Plan goals and outcomes.
  • Conduct shift handovers to ensure continuity of care.
  • Administer medications using Catalyst system and check arrivals during shift.
  • Maintain a clean, safe, and comfortable client environment.
  • Assist guardians and communicate with them as directed by supervisor.
  • Help with personal care tasks and daily living activities.
  • Complete required documentation and meal planning/record-keeping.
  • Perform routine household duties and administrative tasks as assigned.

Description

Hours of work: Every Sun. 11a-11p (24 hrs. biweekly) (CSWI- CR05)

What You’ll Do:

  • Daily supporting clients to work towards their Individual Support Plan - client goals and identified measurable outcomes.
  • Performing a thorough and complete shift exchange with previous shift.
  • Using Catalyst system while administering medications to clients on time and checking/receiving medication that arrive while on shift.
  • Maintaining a clean, safe, and comfortable environment for clients.
  • Making Guardians feel welcome into the client’s residences and communicating/interacting with guardians of clients as directed by the site supervisor.
  • Supporting client’s care needs such as: hygiene, dressing, making beds, toileting, etc.
  • Assisting and monitoring clients through the entire shift.
  • Completing, reading, and maintaining all required documentation.
  • Planning for, preparing, and documenting meals and snacks for clients.
  • Performing other administrative duties such as weekly confirmations, order money, end of the month client service updates, send in occupancy reports, and any other assigned tasks.
  • Performing daily and weekly household duties such as: dusting, washing walls, vacuuming, cleaning, laundry, washing windows, shovelling snow, cleaning bathrooms, washing dishes, etc. ensuring a safe and comfortable environment for clients.

What You Bring:
