Job summary

The Calgary Alternative Community Link (CACL) program, operated by Calgary Alternative Support Services Inc, provides person-centered, community-based supports for adults with disabilities in Calgary. Qualifications

  • Post secondary education in Human Services.
  • Current First Aid/Emergency First Aid including CPR/AED.
  • Police Information Check with a Vulnerable Sectors Search.
  • A valid driver’s license and a vehicle may be asset.
  • Ability to maintain confidentiality.
  • Strong verbal and written communications.

Responsibilities

  • Establish and maintain positive, professional relationships with clients, guardians, and support networks.
  • Provide direct support to an assigned caseload of clients in community-based settings.
  • Participate in the development and implementation of ISPs aligned with CAPCC policy and client outcomes.
  • Ensure supports are person-centered, culturally responsive, and promote autonomy and informed choice.
  • Collaborate with families, guardians, case managers, and other professionals for coordinated service delivery.
  • Support clients in building and maintaining relationships within their communities.
  • Maintain accurate and timely documentation, including case notes in the CASS database.
  • Identify and connect clients to community resources that support their goals and inclusion.
