Job summary

The position reports to the Manager - Patient Safety, supports Quality Assurance Committee activities, and develops safety education and communications.

Qualifications

• Bachelor's degree in a relevant discipline (e.g. nursing, allied health, human factors).
• Minimum of 5 years progressive experience in a large and complex healthcare environment.
• Demonstrated interest in patient safety.
• Experience implementing and integrating system wide initiatives.
• Strong understanding of health care systems, quality improvement and performance measurement.
• Verbal and written communication skills with the ability to facilitate group discussion and learning.

Responsibilities

• Partner with site and program leaders on initiatives related to priority patient safety issues.
• Build capacity for clinical teams to recognize and respond to patient safety hazards, close calls, and adverse events using systems thinking and just culture principles.

The Clinical Safety Leader is part of the Patient Safety team within the provincial AHS Quality and Healthcare Improvement portfolio. This position works in a diverse range of healthcare environments, with staff, medical staff, and leaders at all levels of the organization. There is also interaction with patients and families, in collaboration with clinical teams to improve safety.

Description:

Reporting to the Manager - Patient Safety, the Clinical Safety Leader partners with site and program leaders on initiatives related to priority patient safety issues. This position will build capacity for clinical teams to recognize and respond to patient safety hazards, close calls, and clinical adverse events using systems thinking and just culture principles. The Clinical Safety Leader analyzes patient safety data to understand and prioritize hazards that can lead to patient harm, with the goal of system safety improvement; the role supports leaders with managing a patient safety program for their clinical areas. A primary responsibility of the Clinical Safety Leader is to support the activities of Quality Assurance Committees. This includes providing mentorship and support to the Committee chair(s), conducting quality assurance reviews (QARs) using systems analysis methodology and supporting others to conduct QARs, and reviewing patient safety data to identify trends. Other duties may include development and delivery of patient safety education modules, collaborating with subject matter experts to develop safety communications (e.g. Safety Alerts) and supporting campaigns such as Canadian Patient Safety Week. The Clinical Safety Leader is required to create psychologically safe environments for people to be able to learn from mistakes, set aside their biases, and build trust and effective relationships within interdisciplinary groups as system deficiencies are discussed and opportunities for improvement are identified.

Required Qualifications

Preferred Qualifications

Master’s degree in a health related or business administration discipline preferred. Experience in conducting safety reviews, developing recommendations and tracking implementation of recommendations is an asset. Project management training an asset. Completion QHI Patient Safety Certificate or similar program (e.g. IHI Open School Quality & Patient Safety Certificate, CPSI Canadian Patient Safety Officer certificate).
