Job summary

The Clinical Pharmacist will join a nurse practitioner–led clinic in Sarnia, Ontario, focusing on medication management, expanded-scope services, and infusion therapy. This is a staff pharmacist role with opportunities to grow into leadership and, for the right candidate, future ownership involvement.

You will collaborate with onsite Nurse Practitioners and other healthcare professionals to deliver patient-centered care and advance clinical services within the clinic's model.

Qualifications

• Licensed and in good standing with the Ontario College of Pharmacists, or eligible for Ontario licensure.
• Strong clinical judgment and commitment to patient-centred care.
• Excellent communication and collaborative skills.

Responsibilities

• Practice to the full scope of a pharmacist in Ontario, including medication management, minor ailment assessments, vaccinations, medication reviews, and follow-up care.
• Work closely with onsite Nurse Practitioners and other healthcare professionals to support collaborative patient care.
• Utilize the clinic’s EMR as part of coordinated patient-care planning.
• Identify drug therapy problems and take an active role in developing and implementing solutions.
• Provide comprehensive patient counselling and ongoing medication support.
• Develop knowledge and experience in specialty pharmacy and infusion therapy.
• Support therapy coordination, medication authorization processes, and patient support programs.
• Build collaborative relationships with specialists, primary care providers, hospital teams, and other healthcare partners throughout the region.
• Help develop pharmacist-led clinical services and new patient-care pathways.
• Contribute to the development of pharmacy workflows, policies, and operations as the practice continues to grow.

Our client is developing a unique community pharmacy model in Sarnia, focused on pharmacist-led patient care, specialty pharmacy, infusion services, and close collaboration with other healthcare professionals.

Located within a Nurse Practitioner-led clinic, the pharmacy is being designed to give pharmacists the opportunity to spend more time directly with patients, make meaningful clinical decisions, and practice to the full scope of the profession.

Rather than a traditional high-volume dispensing environment, the pharmacy uses a pharmacist-at-intake approach, with support staff handling tasks that do not require a pharmacist so pharmacists can focus more of their time on clinical care.

Position Summary

The Clinical Pharmacist will initially join the team as a Staff Pharmacist and play an important role in the continued development of the pharmacy and its clinical services.

You will work closely with onsite Nurse Practitioners and other healthcare professionals, providing medication management, expanded-scope services, specialty pharmacy support, and ongoing patient care.
