Where We're Hiring

We’re looking for Clinical Leads in six Alberta communities:

- Lethbridge

- Medicine Hat

- Red Deer

- Grande Prairie

- Fort McMurray

- Cold Lake

You must (ideally) live and work in (or near) one of these communities. This role is all about local leadership and direct connection.

Your Role at a Glance

  • Community Engagement: Build and maintain strong relationships with local partners, health services, and resources to support client-centered care.
  • Direct Home Visits: Deliver hands-on care, education, and occasional coverage for staff absences through scheduled home visits.
  • Team Leadership: Supervise and support a field-based care team, including performance management, and collaboration with internal leadership.
  • Coordination & Travel: Manage documentation and communication tasks while traveling within the community and between client homes.

The Clinical Lead will live and work in their local community, providing direct client care while building and overseeing a multidisciplinary team (e.g., nurses, HCAs, PSWs) in their region. Supported by our client, they’ll play a key role in expanding services and fostering trusted relationships across both formal and informal care networks.

This field-based role offers flexibility to work from home when not in the community. It requires someone with strong people and business acumen, high empathy, and ideally, experience in gerontology and dementia care. Must also do some hands-on clinical field work as they build the team.

A hands-on community health position where collaboration with the formal and informal care networks supports a client and family-centered home care model in the local neighborhood. The role will build a collaborative team based on the principles of trust, autonomy, creativity, and simplicity. The personalized approach to home care is curated for people who wish to age in place their way. This role will build community connections and relationships to provide direct care, within their scope of practice, in client homes, and supervise their team.

Who We’re Looking For

We’re seeking compassionate, community-minded leaders with a blend of clinical expertise and team leadership experience. You bring empathy, initiative, and a practical, hands-on approach to delivering care in the community.

Qualifications & Experience

  • Completion of an accredited healthcare program is required and preferably a regulated health professional including (but not limited to) Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Social Workers (SW), Occupational Therapists (OT), Paramedics (etc.).
  • 5–10 years of progressive experience in community healthcare, elder care, home care, public health, hospitals, or primary care—ideally in field-based or community-centered roles
  • Experience with wellness checks or proactive outreach is strongly valued
  • Proven people leadership skills, including supervision, mentorship, performance management, and team building
  • This role will be held by an individual with strong people and business leadership experiences, ideally with some experiences with gerontology and dementia.
  • Passion for community engagement and building strong local partnerships
  • Track record of supporting high-performing, collaborative teams centered on client and family needs
  • Ideally experience working with adult and geriatric populations, including care planning with families
  • Current registration in good standing with the relevant provincial regulatory body
  • Valid driver’s license, clean driving record, and consistent access to a reliable personal vehicle for community-based work
