Job Title: Clinical Leader

Location: Lethbridge, Alberta

Job Type: Permanent Full-Time

Workplace: Hybrid – 60% in Community

The Clinical Leader of our local Bayshore Neighborhood Health Team (NHT) will have the opportunity to lead a team of healthcare providers supporting clients with in-home care services, working closely with doctors and local health and wellness providers.

This is a community health position where you will provide personalized home care for individuals wishing to age in place. Our nurse-led approach allows nurses to be actively involved in clients’ health and wellness journeys, designing care plans tailored to each client’s lifestyle.

Duties & Responsibilities

• Develop holistic care plans for clients and families to help achieve aging in place goals, adjusting services as needs change.
• Oversee the delivery of in-home services and daily functions of field staff, ensuring access to clinical advice, support, and resources.
• Hire, train, and supervise healthcare staff; ensure excellent interactions with clients; provide coaching and performance feedback.
• Manage expectations with families and clients, ensuring a client-centered approach to care planning.
• Adapt care plans to changing client needs, including collaborating with interdisciplinary teams for acute issues.
• Ensure shift coverage, including providing direct care when necessary.

Minimum Qualifications

• Current and valid RN license in British Columbia
• At least 5 years of healthcare experience, preferably in community settings
• At least 2 years of leadership experience managing teams
• Current CPR certification
• Experience with community healthcare, geriatrics, and families is preferred
• Valid driver’s license and reliable vehicle for home visits (vehicle allowance provided)
