Job summary

SE Health in Goderich, ON is seeking nursing professionals to provide comprehensive client care in a clinic setting. You will assess, plan, implement, and evaluate care based on best practices and the client’s needs, while collaborating with a multidisciplinary team.

The role requires active registration, CPR, and the ability to work 2–3 days per week with weekend rotation, in a dynamic, supportive environment that values growth and full scope practice.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work 2–3 days per week, every third weekend, on-call rotation.
• Willingness to attain SE Health certifications.
• Current CPR certificate is required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and client needs.
• Act as primary provider for a client caseload and lead care when designated, collaborating with the care team.
• Assist clients through transitions by linking to community support services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client's unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work 2-3 days per week, every third weekend, participation in the on-call rotator. This role is at Goderich Clinic 181 Cambria Rd N.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
