Job summary

SE Health in Ontario is seeking a part-time registered nurse (RN) or registered practical nurse (RPN) to join a supportive team, with 2-6pm shifts and 122 King St, Hensall, ON. The role emphasizes comprehensive client assessments, care planning, and collaboration with the care team to deliver high-quality home or community-based care.

Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario.
• Availability to work PT Monday-Friday, 2-6pm at 122 King St in Hensall, ON.
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications.
• A current CPR certificate is required.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
