Job summary

SE Health in Ontario is hiring registered nurses to provide community-based care. The role emphasizes comprehensive assessments, care planning, and coordination with the care team to support clients in home and community settings.

We offer flexible shifts, paid training, and opportunities for professional growth alongside a supportive team. The compensation includes paid per-visit pay range between CAD 36.35 and CAD 40.35, with travel required.

Qualifications

• Current registration with the College of Nurses of Ontario.
• Canadian Nurses Association specialty certification an asset.
• CPR certificate required.
• Valid driver’s license and vehicle; travel required.
• Updated immunization records and background check required on hire.
• Willingness to attain Saint Elizabeth certifications.
• Availability for days, evenings, overnights, weekends and on-call shifts.
• Physically demanding role with lifting up to 55 pounds.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence and client needs.
• When designated as primary provider for a client caseload, be the point of care lead with the care team.
• Support the client through care transitions by connecting with community support services.

Responsibilities:

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements:

• Current registration with the College of Nurses of Ontario
• Canadian Nurses Association (CNA) specialty certification an asset
• Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
• A current CPR certificate is required
• A valid driver’s license and vehicle (travel required)
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required Saint Elizabeth certifications.
• Availability to work a mix of days, evening and overnight, weekends and on call shifts.
• This is a physically demanding position and requires hand, arm and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement
