Qualifications

  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Willingness to attain SE Health certifications.
  • Current CPR certificate is required.
  • Availability to work PT Monday-Friday, 2-6pm at 122 King St, Hensall, ON.
  • Updated immunization records and clear background checks required.

Responsibilities

  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence.
  • When designated as primary provider for a client caseload, lead care and collaborate with the team.
  • Support clients through transitions by connecting with community support services.

Responsibilities
  • Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs
  • When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
  • Support the client through care transitions by collaborating and connecting the client with community support services in their local community.
Requirements
  • Current Certificate of Competence with the College of Nurses of Ontario.
  • Availability to work PT Monday-Friday, 2-6pm at 122 King St in Hensall, ON.
  • Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
  • A willingness to attain required SE Health certifications
  • A current CPR certificate is required.
  • This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
