Job summary

SE Health in Fergus, Ontario, is hiring a Registered Nurse to assess clients, create care plans, and coordinate transitions. You will lead a client caseload as the primary provider in collaboration with the care team, traveling to local areas as needed.

Requires RN with Certificate of Competence, availability for full-time days with every other weekend, on-call shifts, and up-to-date immunizations and background checks. SE Health offers strong benefits and professional growth opportunities.

Responsibilities

• Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
• When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
• Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements

• Current Certificate of Competence with the College of Nurses of Ontario as RN.
• Availability to work FT days with every other weekend and on call component (Evening and Overnight 6 nights/month.)
• Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
• A willingness to attain required SE Health certifications
• A current CPR certificate is required.
• This is a physically demanding position and requires sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level (i.e. shoulder flexion), and numerous hand grips of varying forces.
• Required to travel to Fergus/Elora/Drayton/Arthur and surrounding areas.
