Emergency preparedness is a critical aspect of CCH operations. It is essential that all staff and physicians receive training and understand their roles and responsibilities in this field. This position is for an existing vacancy.
Reporting to the Chronic Disease Management (CDM) Clinical Manager, the CDM Registered Nurse (RN) will work as part of the interdisciplinary team to assess, care for, and educate chronic disease management patients, their families, and caregivers in an outpatient clinic setting.The primary role of CDM RN at Cornwall Community Hospital (CCH) is to deliver safe, effective, patient-centred, ethical, and evidence-based nursing care. The overall function of the CDM RN is to assist patients and the community in managing clinical conditions and transitions while maximizing their self-care abilities. The CDM RN will also serve as a resource for care team members, supporting them across the CDM program to facilitate exceptional patient care.
• Current Certificate of Competence from the College of Nurses of Ontario (CNO)
• Current Certification in Basic Cardiac Life Support (BCLS)
• Demonstrated participation in continuing education programs, seminars or other related activities to maintain knowledge of chronic disease management nursing.
• Knowledge of Quality Improvement
• Regular, punctual attendance
• Ability to meet the physical demands of the position
• Functional level in both official languages (oral and written in English; oral in French, and testing completed)
• Relevant experience in an outpatient/ambulatory clinic setting
**Must demonstrate at least three of the following assets:
Certified Diabetes Educator (CDE) and Diabetes Educator Diploma
Insulin Pump Expertise or Insulin Pump Trainer Certification
Telehealth experience
Certification in Peripherally Inserted Central Catheter (PICC)
Respiratory Care Certificate
Special training in teaching and counselling skills, such as Motivational Interviewing
• Superior interpersonal, organizational, and communication skills
• Ability to work within an interdisciplinary team
• Ability to work well under pressure and to adapt to change
• Excellent patient assessment, critical thinking, communication (oral and written), and problem-solving skills
• Ability to develop, implement, evaluate and adapt a therapeutic nursing plan (Care plan)
• Ability to teach and mentor nurses and students as required
• Commitment to personal continuing education activities, including self-evaluation
• Demonstrates leadership decision-making skills and abilities
Commitment to patient safety
• Working knowledge of Cerner electronic health record (or alternative electronic health record)
• Excellent computer skills and proficiency in Microsoft Office, email, internet, etc.
