Job summary

Halton Healthcare seeks a Child and Youth Worker for the Navigator Program in Outpatient Mental Health. You will collaborate with inpatient and outpatient teams to support pediatric patients in the community and school system.

You will conduct home/community visits, address housing and financial issues, and assist with daily living skills, school accommodations, and links to services. Demanding but rewarding role in a dynamic care environment.

Qualifications

• CYW certificate and diploma or degree from a recognized community college or university.
• 3-5 years of child and adolescent experience preferably in hospital setting.
• Knowledge of children's psycho-social development.
• Ability to work independently and effectively within a team.
• Competency in assessing risk issues and addressing concerns within the community.
• Experience with high conflict families and de-escalation skills.
• Understanding of the school system and Halton community supports.
• Computer skills (Word Processing).
• Satisfactory employment and good attendance record.

Responsibilities

• Collaborate with inpatient and outpatient teams on treatment plans for pediatric patients in the community.
• Conduct home and community visits to address school compliance and treatment follow-through, providing family support and linking to services.
• Assist patients with housing, finances, daily living skills, scheduling, and accessing supports.
• Accompany patients to school when needed and coordinate with the Mental Health Liaison Nurse/Social Worker for school accommodations.
• Support seamless transitions between inpatient and outpatient teams and help patients engage with appropriate services.
• Support patients involved with the Emergency Department and ensure timely connection to services.

Working closely with the inpatient and outpatient child and adolescent interdisciplinary teams the Child and Youth Care (CYC) worker will collaborate on treatment plans for patients in the community, conduct home and community visits to address issues such as school compliance, or lack of follow through with office treatment and provide family with support while connecting them to appropriate services.

The CYC will work closely with the patient to address any housing issues, financial struggles and ensure that they have adequate skills for daily activities, such as grocery shopping, attending appointments, banking/ budgets, applying for appropriate supports, such as student welfare etc.The worker will accompany patients to school if school compliance is an issue and will work closely with the Mental Health Liaison Nurse/ Social Worker to ensure the patient is receiving appropriate school accommodations.

From the inpatient unit, the CYC will assist patients by integrating them back into the school system to ensure a successful transition. For patients involved with our outpatient mental health program who are struggling with socializing with their peers due to their mental illness, the CYC will engage them in social activities such as attending the YMCA.

The CYC will work closely with the Child Psychiatrists, Social Workers and Nurses to ensure that progress or delay in treatment is articulated to ensure a seamless transition between the teams.The CYC will also play an important role for patients who attend the Emergency Department in ensuring that the patient and their family are connected with in a timely fashion and supported until engaged to appropriate services.

Salary:$36.667 to $44.796 per hour
