Halton Healthcare is seeking a qualified social worker for the Adult Outpatient Mental Health Program in Oakville, Milton or Georgetown sites. You will assess, plan, implement and evaluate treatment for individuals with concurrent disorders, coordinating with a multi-disciplinary team and community partners.
The role requires flexible hours, some evenings, and travel between sites. This is a temporary full-time position with an approximately one-year contract and shift pattern as described.

Working in the Adult Outpatient Mental Health Program, the candidate will be required to complete assessments, develop, implement and evaluate treatment plans for individuals with complex concurrent disorders (mental health and addiction). The candidate will liaise with other services within the Mental Health program and with community agencies to coordinate care for clients. They will be expected to utilize critical thinking, problem solving and customer service in their approach to care and service delivery. In addition, they will work with an inter-professional team and participate in program development and evaluation, internal/external presentations and continuous quality improvement activities. The candidate will support a workplace that embraces diversity, encouraged teamwork and complies with all applicable regulatory requirements. Ability to work flexible hours is a requirement and some evening hours will be required.

Hours:Monday to Friday - 7.5 hour shifts - 08:30 - 16:30, except Wednesday which an evening shift 1100-1900 (hours subject to change based on operational needs)
Salary: $47.039 to $60.979 per hour
