Job summary

SLCS is seeking a Full-Time Case Worker for the Supported Independent Living (SIL) program in Ontario. You will provide support services, crisis intervention and connect adults with developmental disabilities to resources and life skills education within the community.

You will develop case plans, assess risk, coordinate with partners, document notes, and travel between facilities and community locations. A 12-hour shift rotation and every-other-weekend 8-hour shift are required.

Qualifications

• Post-secondary education from an accredited institution.
• 1-3 years’ work experience dealing with people with developmental disabilities, acquired brain injuries, addictions services, and/or fetal alcohol spectrum.
• 1-3 years’ experience working with people who may become verbally or physically aggressive, using de-escalation strategies to mitigate risk.
• Standard First Aid/CPR with AED - Level C.
• Valid G Class Driver’s License and access to a reliable vehicle with appropriate insurance is required.
• Case management experience is an asset.
• Proficiency in both official languages of Canada is an asset.

Job description

SLCS is currently seeking Full-Time Case Workers for our Supported Independent Living (SIL) Program. The people we support live within the community and receive assistance with connection to resources, life skills education and activities of daily living. As a member of this interdisciplinary team, you will provide support services and crisis intervention to adults with disabilities, mental health needs and/or addictions. This role may support people who have the potential to become verbally or physically aggressive in a one to one setting.

Responsibilities of this position include:

• Facilitate development of individual case plans with adults who have developmental disabilities and are at a risk of or currently involved in criminal justice system who require support to live independently
• Connect clients to community supports and resources in order to achieve their current goals
• Educate clients on various life skills to facilitate their independence
• Assess risk and need of clients in relation to community safety and care plans
• Liaise with other team members to coordinate support of our clients
• Regularly travel in your own vehicle between SLCS facilities and community locations
• Establish and maintain professional working relationships with community organizations
• Documenting and preparing case notes
• Connect with client’s family and supports to remain engaged and assist as a resource for the wellbeing of our clients

The program runs between the hours of 8am - 10pm, 7 days a week. The successful candidate will be required to work a rotation of 12 hour shifts as well as one 8 hour shift every two weeks on a Saturday. The rotation includes a 3 day weekend every other weekend and a complete sample schedule will be provided during the pre-screening phase.
