Reporting to the Manager, Developmental Services- Adult and PDP, the Case Manager, Alternative Level of Care provides intensive, time-limited case management for adults with developmental disabilities who have been designated Alternative Level of Care (ALC), are at risk of inappropriate hospitalization, or require coordinated transition planning to live safely in the community. The role supports person-directed planning, timely discharge, service coordination and access to appropriate community-based supports. Working closely with clients, families/caregivers, hospitals, developmental services, health partners and community agencies, the Case Manager helps reduce system barriers, strengthen continuity of care and promote dignity, inclusion and quality of life.
