Job summary

St. Leonard’s Community Services (SLCS) is seeking a Community Case Worker for the Reintegration Program on an 18 month contract.

You will play an integral part of assessment, case management, day-to-day supports and discharge planning for participants who may have involvement with the criminal justice system and mental health/addiction challenges. You will develop individualized support plans, network with partners, assess risk, and support reintegration into the community.

Qualifications

  • Post secondary education from an accredited institution.
  • 1 - 3 yrs relevant experience in social services.
  • First Aid/CPR with AED - Level C.
  • Valid G Class Driver’s License and a reliable vehicle.
  • Bilingual in English and French will be considered an asset.

Responsibilities

  • Assess and manage caseload for participants in the Reintegration Program.
  • Develop and implement individualized supports and discharge planning.
  • Coordinate with SLCS psychiatric supports and community partners.
  • Liaise with ministry, court and police officials as needed.
  • Represent SLCS in the community and travel between facilities and locations.

Job description

St. Leonard’s Community Services (SLCS) is currently seeking a Community Case Worker for the Reintegration Program on an 18 month contract. You will play an integral part of the assessment, case management, day-to-day supports and discharge planning for community referred individuals who may have current/recent involvement in the criminal justice system, and who may also experience mental health and/or addiction challenges. As a team member, you will be responsible to develop and implement individualized support plans for participants while supporting their reintegration into the community. You will be responsible to network and develop partnerships to promote the delivery of services and connect program participants to their communities of choice. You will also be responsible for assessing risk and clinical suitability for participants who are in various community locations, including connection with SLCS’ psychiatric supports, as appropriate. You will liaise with other community partners and team members to provide consultation for clients with an expertise in high-risk, mental health participants as well as having strong advocacy skills and confidence in your ability to manage crisis situations. You will have the ability to work independently in the community, with a focus on multitasking and be able to change priorities as needed. You will be expected to represent SLCS in the community and liaise with ministry, court and police officials. This position would be working Monday - Friday, 8am - 4pm You will be able to work with staff and clients from diverse populations; contribute to program development; and provide support to co-workers as a member of an interdisciplinary team. You will participate in a workplace that embraces diversity, encourages teamwork and complies with all applicable and regulatory requirements. This position is located in the community and you will be expected to travel between the residential facilities and work within various community locations.

Required Qualifications
  • Post secondary education from an accredited institution
  • 1 - 3 yrs. relevant experience in social services
  • Intermediate / Standard First Aid /CPR with AED - Level C
  • Valid G Class Driver’s License and a reliable vehicle
  • Candidates who are proficient in both official languages of Canada will be considered an asset
