Job summary

The Case Manager at Autism Services will support individuals involved in high-risk lifestyles by building on strengths and coordinating with health, social, and community services to meet daily living, health and financial needs.

You will provide one-on-one support, work with families in their homes and in Saskatoon neighborhoods, and collaborate with other professionals to connect clients with essential resources, housing, and community programs, including after-hours support as needed.

Qualifications

  • Bachelor's degree in Social Work or related field required.
  • Ability to speak a First Nations or Mtis language is an asset.
  • Knowledge of Aboriginal histories and backgrounds.
  • Understanding of barriers to healthy living linked to high-risk lifestyles.
  • Must possess and maintain a valid Class 5 driver's license and own a reliable vehicle.
  • Criminal and vulnerable persons record checks required.

Responsibilities

  • The Case Manager will support individuals involved in high-risk lifestyles by coordinating with professionals and agencies to meet daily living, health, financial and social needs.
  • Provide one-on-one support and collaborate with families within homes and in Saskatoon neighborhoods.
  • Coordinate with service providers to access housing, health care, finances and community resources.
  • Adapt to non-traditional hours including evenings and weekends.

Position Profile: The Case Manager will work to support individuals who have been involved in a high-risk lifestyle, with the philosophy of building on strengths and supporting challenges. This will require providing one on one support as well as working with various professionals and support agencies to coordinate meeting the daily living, health, financial and social needs of individuals and/or families who have been involved in high-risk lifestyle.

QUALIFICATIONS
  • Bachelors degree in Social Work or related field of study and/or a suitable combination of training and extensive experience in working with individuals who have been involved in a high risk lifestyle.
  • Ability to speak a First Nations or Mtis language is an asset.
  • Knowledge and understanding of the history of Aboriginal peoples and their socio-economic backgrounds.
  • An understanding of the barriers to healthy living that can contribute to higher risk for child abuse and neglect.
  • An understanding of child development and age-appropriate developmental needs of children.
  • Must possess and understanding of positive and culturally appropriate approaches to parenting.
  • Must be knowledgeable of the dynamics of living with HIV/AIDS.
  • Must be knowledgeable with Fetal Alcohol Spectrum Disorder and other cognitive disabilities.
  • Must have knowledge of both solutions focused and strength-based theories.
  • Knowledge of alcohol/drug recovery and relapse dynamics is an asset.
  • Awareness of the services and resources available within the community of Saskatoon.
  • Must be comfortable working with and providing intensive supports to families within their own homes and in the core neighborhoods of Saskatoon.
  • Willing and available to work non-traditional hours such as evenings and weekends.
  • Criminal and vulnerable persons record check.
  • Must possess a valid Class 5 drivers license and own a reliable vehicle.
  • Must have a working knowledge of windows-based computer software programs.
