The Outreach Worker provides community-based, client-centered support to individuals experiencing or at risk of homelessness, substance use, and HIV, using a harm reduction and trauma-informed approach. Working as part of a two-person outreach team, they engage individuals in the community including on the streets, in shelters, public spaces, and in housing to build trusting relationships and reconnect them to essential services such as housing, healthcare, HIV supports, and basic needs. The role includes conducting regular wellness checks to support safety and housing stability, connecting participants to internal programs and community resources, and supporting access to medical care through referrals, accompaniment, and transportation as needed. Outreach Workers also provide harm reduction supplies and basic needs supports, assist in the organizations drop-in centre when required, and encourage individuals to work toward personal goals such as sobriety, housing stability, employment, or education when they are ready. The position requires maintaining accurate documentation, participating in team meetings and case conferencing, and contributing to a collaborative, responsive approach to care.
