Job summary

Ottawa Salus Corporation in Ottawa is seeking a Case Manager to provide community-based support to adults with severe mental illness. You will assess needs, develop service plans, and refer clients to community resources to support recovery.

The role emphasizes a portable, client-centered approach across housing, health, finances and daily living, with a focus on individualized care and advocacy.

Qualifications

• Bachelor’s degree in Social or Health services, related field or equivalent.
• At least three years of experience working with adults who have a psychiatric disability.
• Suicide Prevention Training.
• Non-Violent Crisis Intervention.
• Knowledge of community mental health resources in Ottawa.
• Knowledge of housing, financial, mental health, physical health and other community resources and referral procedures.
• Knowledge of psycho-social rehabilitation and case management.

Responsibilities

• Assess client needs and develop a service plan.

Job Description:

The Case Manager provides community-based support to individuals suffering from severe and persistent mental illness. Client needs are assessed, a service plan is developed and the client is referred to appropriate community resources. The service is flexible, portable, individualized and client-centered. The Case Manager provides assistance in the following life domains: basic needs, daily living, physical and mental health, housing, financial, social, spiritual, legal, educational and vocational.

Qualifications:

• Valid drivers’ license and use of own vehicle with appropriate insurance is required.
