Job summary

BayCare Health System in Florida is seeking a Case Management Assistant to support patient and family access to services. You will assist with transportation, follow-up appointments, and coordinating care in ambulatory settings, acting as the first point of contact for the department.

The role requires at least 1 year in a medical-related field or customer service and strong interpersonal and communication skills. Schedule varies, with days 7:30 AM–4:00 PM and occasional weekends; PRN, as needed.

Qualifications

• Required High School or Equivalent.
• Preferred Associate's degree.
• 1 year in Medical Related Field or 1 year of Customer Service experience.
• Excellent written and verbal communication.

Responsibilities

• Assist with completing tasks to facilitate patient and family access to services.
• Support care coordination by handling inbound calls and patient needs in ambulatory settings.
• Serve as initial department contact and route calls to the appropriate discipline.
• Perform other duties as assigned.

The Case Management Assistant Responsibilities Include

• Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, follow up appointments, and services
• Responsible for several activities including facilitating inbound calls and assisting patients' needs to support successful care coordination in the ambulatory setting.
• Responsible for customer service as the initial contact for the department and quickly determines the needs of the patient to elevate calls professionally and patiently by routing patients to the appropriate discipline.
• Performs other duties as assigned.

• Status: PRN, As Needed
• Schedule: Days Vary, 7:30 AM - 4:00 PM
