Job summary

Qualifications

• Three to five years of NP experience; primary care preferred.
• Leadership/program management/quality improvement experience.
• Strong clinical assessment and decision-making within NP scope.

Purpose of the Position:

CMHA Muskoka-Parry Sound is seeking a Nurse Practitioner to provide comprehensive primary care and day-to-day clinical and operational leadership for CareBridge, a collaborative West Parry Sound initiative supported by the West Parry Sound Ontario Health Team. CareBridge provides timely interim primary care for people who do not currently have a regular primary care provider while supporting safe, coordinated pathways to ongoing attachment. The CareBridge NP Manager combines direct Nurse Practitioner practice with program leadership, working closely with the Clinic Coordinator, regional primary care providers, CMHA MPS, the OHT and community partners. Responsibilities include assessment, diagnosis and treatment; health promotion and disease prevention; care coordination and transitions; day-to-day clinic operations and team workflows; access and attachment pathways; quality improvement and program performance; and collaborative relationship-building across the local health system.

Education and Qualifications:

• Current certificate of registration in good standing with the College of Nurses of Ontario in the Extended Class.
• Bachelor of Science in Nursing (BScN) from a recognized university and Primary Health Care Nurse Practitioner certification or equivalent recognized preparation.
• Three to five years of relevant Nurse Practitioner experience; primary care experience is strongly preferred.
• Demonstrated leadership, program management, quality improvement or supervisory experience.
• Strong clinical assessment, diagnostic, therapeutic and decision-making skills within Nurse Practitioner scope of practice.
• Ability to work autonomously as well as collaboratively within interdisciplinary and inter-organizational teams in a rural setting.
• Excellent communication, organization, leadership, problem-solving and interpersonal skills.
• Demonstrated commitment to equitable, trauma-informed, person-centred and culturally safe care.
• Indigenous Cultural Awareness and Education is a requirement, together with a commitment to ongoing learning.
• Bilingual in French/English is considered an asset.
• Strong digital literacy, including experience with electronic medical records and standard office technology.
• A valid Ontario G or G2 driver's licence, a safe and reliable means of transportation and appropriate insurance for work purposes with a minimum of $1,000,000 in liability coverage.
• Primarily daytime Monday to Friday; flexibility or extended hours may be required based on program needs.

Salary scale: $67.55 to $78.31 per hour, commensurate with qualifications and experience

Benefits include: Enrollment in a Registered Retirement Plan with HOOPP where contributions start on date of hire. Other benefit eligibility will be in accordance with CMHA MPS policy and the terms of the position.
