Job summary

Canadian Mental Health Association Muskoka–Parry Sound (CMHAMPS) is seeking a Clinic Coordinator to support CareBridge operations. The role focuses on clinic administration, intake and referrals, client communication, and EMR support in a 37.5-hour week on a contract basis.

You will work closely with the NP Manager, regional primary care providers, and community partners to ensure timely, coordinated pathways and attachment for clients in the West Parry Sound area.

Qualifications

• Post-secondary education in medical office administration, health administration, social service work, community health, nursing, mental health, or a related field is preferred.
• Experience in a healthcare, primary care, community health, mental health, social services, or clinic administration setting.
• Experience with intake, referral management, appointment scheduling, client communication, documentation, follow-up processes, and health or social service navigation.
• Strong digital literacy, including experience using electronic medical records, Microsoft Office, and other digital tools. Experience with PS Suite and/or Ocean is an asset.
• Understanding of privacy, confidentiality, consent, and secure handling of personal health information.
• Comfort working in a developing program environment, adapting to evolving workflows, and contributing to testing and refining clinic processes.
• Ability to work autonomously as well as collaboratively within interdisciplinary and inter-organizational teams in a rural setting.
• Excellent communication, organization, problem-solving, and interpersonal skills.
• Indigenous Cultural Awareness and Education is a requirement.
• A bilingual French/English language considered an asset
• A valid Ontario G or G2 driver's licence with liability coverage.

Responsibilities

• Clinic administration, intake and referral processes, client communication, documentation, EMR and digital workflow support.
• Attachment tracking, reporting, Health Care Connect waitlist reconciliation, and partner follow-up.
• Support ongoing coordination with CareBridge Nurse Practitioner Manager, regional primary care providers, and partners.

Purpose of the Position:

CMHA Muskoka-Parry Sound is seeking a Clinic Coordinator to support the day-to-day operation and coordination of CareBridge, a collaborative West Parry Sound initiative supported by the West Parry Sound Ontario Health Team (OHT). CareBridge provides timely interim primary care for people who do not currently have a regular primary care provider while supporting safe, coordinated pathways to ongoing attachment. The CareBridge Clinic Coordinator works closely with the CareBridge Nurse Practitioner (NP) Manager, regional primary care providers, CMHA MPS, the OHT, Ontario Health atHome and community partners. Responsibilities include clinic administration, intake and referral processes, client communication, documentation, EMR and digital workflow support, attachment tracking, reporting, Health Care Connect waitlist reconciliation, and partner follow-up.

Education and Qualifications:

• Post-secondary education in medical office administration, health administration, social service work, community health, nursing, mental health, or a related field is preferred. Candidates with significant directly related experience in clinic administration, intake, referral management, or client service may also be considered.
• Experience in a healthcare, primary care, community health, mental health, social services, or clinic administration setting.
• Experience with intake, referral management, appointment scheduling, client communication, documentation, follow-up processes, and health or social service navigation.
• Strong digital literacy, including experience using electronic medical records, Microsoft Office, and other digital tools. Experience with PS Suite and/or Ocean is an asset.
• Understanding of privacy, confidentiality, consent, and secure handling of personal health information.

• Primarily daytime Monday to Friday; flexibility or extended hours may be required based on program needs.

Salary scale:

Salary scale: $28.00 to $39.00 per hour, commensurate with qualifications and experience
