About the Role
A full-time remote Care Coordinator is being sought to serve as the first voice a patient—or their family—hears when reaching out for care. In this role, the Care Coordinator will guide older adults and their caregivers through the intake process with warmth, clarity, and professionalism, ensuring they feel supported every step of the way.
This position is a unique blend of patient support, care coordination, and emotional guidance. The Care Coordinator will be the steady hand helping each new patient—and their family—navigate the journey into care, from matching with the right therapist to ensuring they feel confident and understood every step of the way. This role is ideal for someone who thrives at the intersection of empathy and execution, and who finds meaning in turning a moment of vulnerability into one of reassurance and trust.
This opportunity is especially well-suited for someone with an interest in psychology or mental health, as the Care Coordinator will often be the first emotional point of contact for individuals seeking care.
What the Care Coordinator Will Do
- Be the front line: Serve as the primary point of contact for new patients and their families—answering questions, sharing information about available services, and guiding them through next steps
- Coordinate onboarding: Schedule consultations, send reminders, and ensure all intake paperwork is completed promptly and correctly
- Support smooth handoffs: Match patients with therapists based on availability and preferences, and ensure the care team is informed with all relevant details
- Track engagement: Follow up with patients who miss sessions or need rescheduling, and flag issues to the clinical or operations team as needed
- Represent the mission: Speak with empathy and clarity to patients and caregivers, helping reduce stigma and build trust from the very first interaction
Who the Ideal Candidate Is
- Empathetic and clear communicator. Speaks with kindness, listens actively, and makes others feel at ease—even during difficult or emotional conversations.
- Highly organized and detail-oriented. Able to juggle multiple patients at different stages in the process and never lets things slip through the cracks.
- Tech-comfortable. Comfortable learning tools like Google Workspace, EHR platforms (e.g., Healthie), or CRMs like HubSpot, and picks up new software quickly.
- Mission-aligned. Cares deeply about improving care for older adults and is excited to play a key role in someone's first step toward better mental health.
- Experience welcome, not required. May have worked in healthcare, customer service, or an administrative support role before—but most importantly, is organized, compassionate, and eager to grow.
