Job summary

CBI Home Health in Hamilton is seeking a Care Access Representative to coordinate scheduling for Personal Support Workers, Health Care Aides and Nurses in a high-volume contact center.

You will manage chats, inbound/outbound calls, data entry and intake, respond to scheduling requests, resolve issues, and ensure continuity of care. This is an on-site, full-time role with a flexible environment and comprehensive rewards.

Qualifications

• Minimum Grade 12 education.
• At least 2 years of experience in Customer Service, Scheduling, and/or Coordination, preferably within a contact center environment.
• Proficiency in Microsoft Office Suite and scheduling software (e.g., AlayaCare). Keyboarding speed of 35-45 words per minutes.
• Strong verbal and written communication skills, with excellent telephone etiquette.
• Problem-solving, multitasking, organizational skills and attention to detail.

Responsibilities

• Efficient Scheduling: use scheduling software to optimize care provider assignments.
• KPI Achievement: meet and exceed KPIs for calls/chats per hour and scheduling success.
• Stakeholder Communication: maintain open lines with clients, families and care providers.
• Relationship Building: develop positive relationships with clients and providers.
• Data Management: update scheduling databases and care plans accurately.
• Compliance: adhere to employment standards and organizational policies.
