Ontario Health atHome is seeking a Care Coordinator in Bancroft, ON. You will assess patient care needs, determine eligibility, and connect patients with community services while collaborating with patients, families and health-care teams.

You will coordinate and monitor care plans, establish relationships with families, and respect patient values as you balance resources in a busy multidisciplinary setting. A strong background in community health and documentation is essential.

Qualifikationen

• University degree in Nursing, Physiotherapy, Occupational Therapy, Speech Language Pathology, Social Work or related field.
• 2+ years of recent experience in community health or related field.
• Knowledge of health care delivery system and community resources.
• Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
• Effective time management and organizational skills; able to work independently in a busy multidisciplinary environment.
• Ability to complete required documentation and reports accurately.
• Valid driver’s licence and access to reliable vehicle.
• Proficient in Windows.
