Qualifications

• Membership in good standing with the applicable regulatory body (Nursing/Physiotherapy/OT/Social Work)
• Recent experience in community health or related field
• Excellent interpersonal, communication, assessment, problem-solving and decision-making skills

Responsibilities

• Assess care needs and determine eligibility for services
• Link patients with service providers and coordinate care plans
• Monitor care plan delivery and maintain documentation
• Establish relationships with patients and families
• Balance needs with available resources while respecting patient preferences
• Initial intake and potential assignment changes as needed
